Thank you for looking into joining our surgery for your health care. We ask you to read the instructions below carefully to ensure your registration is processed correctly with minimum delay.
IN ORDER TO REGISTER YOU CAN EITHER COME IN TO THE SURGERY AND COLLECT THE FORMS, TAKE THEM HOME, COMPLETE THEM AND THEN BRING THEM BACK.
YOU CAN PRINT OFF THE ATTACHED FORMS BELOW, COMPLETE THEM AT HOME AND THEN BRING THEM TO THE SURGERY
IF YOU ARE A BIG FAMILY OF MORE THAN 2 PEOPLE, THEN WE ASK ONLY 1 MEMBER OF THE FAMILY TO BRING IN THE FORMS TO US PLEASE TO REDUCE FOOTFALL AND MINIMISE RISK OF COVID INFECTION TRANSMISSION. MORE THAN 2 PEOPLE WILL BE ASKED TO WAIT OUTSIDE.
We will not be accepting new registration paperwork or forms via email.
Please complete both forms with your NHS number provided (this is very important as it helps us to find you in the NHS spine and we can then access your records) and bring one proof of ID and one proof of address with you if possible.
Please allow at least 2 weeks to complete the registration.
We will request you to submit your ID and proof of address. However you will not be refused to register if you do not have access to this so please speak to reception for advice.
Acceptable Identification; tenancy agreement, mortgage agreement, bank statement, driving licence, passport, birth certificate etc.
If you have registered before with the NHS you will not be required to have a new patient health check. However if you are new to the country and never been with the NHS, please read on.
Once registered, make an appointment with our nursing team for a registration medical.
We invite all our new patients to see one of our practice nurses or our health care assistant. This appointment enables us to collect medical information before your medical records arrive. It is also the start of screening for other medical problems. Normally, you will need to complete a registration medical before seeing one of our GPs.
For the Donor registration from 20 May 2020 this is automatic unless you OPT OUT and you need to call 0300 123 2323 to opt out. More information is on the link below:
